
CONGRATULATIONS PARKTREE for receiving the Community Health Investment Fund (CHIF) Oral Health Initiative XVI grant in the amount of $100,000.00. The purpose of the grant is to leverage the Electronic Dental Record along with other population health tools to develop a tracking and recall system to monitor care gaps and enhance educational strategies for at least 2,100 patients. The goal of this project is to increase treatment completion rates and prevent periodontal disease. This project is being implemented for those patients seeking care at Holt Dental center located at 1448 Holt Avenue Pomona, CA.
